How to Apply
Your Next Step Starts Here
Whether you're applying for yourself or referring a client, our intake process is designed to be clear, respectful, and straightforward. Here's everything you need to know.
Eligibility
Who Is the Program For?
Next Phase Housing serves adult men who are experiencing housing instability and are ready to engage with a structured program. We work with men at varying levels of need — from those requiring intensive support to those who are nearly ready for independent living.
We evaluate each applicant individually. If you're unsure whether you or your client qualifies, reach out — we're happy to talk it through.
General eligibility criteria
The intake process
What to Expect, Step by Step
We've designed our intake process to be transparent and supportive. Most applicants complete intake within 3–5 business days.
Submit an Inquiry
Day 1Complete our online inquiry form or call us directly. You'll share basic contact information and a brief description of the housing situation. A staff member will follow up within one business day.
You don't need to have everything figured out before reaching out. We'll help you from here.
Initial Phone Screening
Days 1–2A staff member will conduct a brief phone screening — typically 15–20 minutes — to gather more information about current housing status, support needs, and program fit. This is a conversation, not an interview.
For referral partners: you're welcome to participate in this call alongside your client.
Needs Assessment
Days 2–3If the initial screening indicates a good fit, we'll schedule a more in-depth needs assessment. This helps us understand the full picture — housing history, support needs, goals, and any barriers — so we can match the applicant to the right program tier.
Bring any relevant documentation if you have it — ID, benefits information, prior housing records. We'll work with what's available.
Program Placement Decision
Days 3–5Based on the assessment, our team determines the appropriate housing tier and develops an initial service plan. We'll communicate the decision clearly and walk through next steps together.
If a bed isn't immediately available, we'll add you to our waitlist and stay in contact.
Move-In & Orientation
Upon placementOnce placement is confirmed, we'll schedule a move-in date and conduct a program orientation. You'll meet your case manager, review your service plan, and get settled into your new home.
We'll make sure you know exactly what to bring and what to expect on move-in day.

Common questions
Frequently Asked Questions
Is there a cost to residents?
Program fees vary by tier and are based on ability to pay. We work with residents to identify funding sources including benefits, subsidies, and other supports. No one is turned away solely due to inability to pay.
How long do residents stay in the program?
Length of stay varies based on individual needs and progress. Most residents remain in the program between 6 and 24 months, with the goal of transitioning to permanent independent housing.
What documentation is required for intake?
We ask for a government-issued ID when available, but we will not deny services solely due to lack of documentation. We can help residents obtain ID and other documents as part of our services.
Do you accept referrals from outside the area?
We primarily serve men in our local service area. If you're outside our area, contact us and we'll do our best to connect you with appropriate resources.
What happens if a resident isn't ready for the program?
If someone isn't a fit for our program right now, we'll work to connect them with other resources and keep the door open for the future. We don't turn people away without offering an alternative path.
