Structured Support at Every Level

Next Phase Housing offers a tiered program designed to meet men where they are — providing the right level of housing, services, and accountability for each stage of the journey toward independence.

A Program Built Around Real Needs

We recognize that men come to us from different circumstances and with different levels of need. That's why our program is structured in tiers — each one offering a distinct combination of housing stability, case management intensity, and personal accountability. As residents grow and demonstrate readiness, they move through the tiers toward fully independent living.

Housing tiers

Three Tiers, One Pathway Forward

Tier 1Most structured

High-Support Housing

Designed for men who need the most support — including those transitioning from shelters, hospitals, or incarceration. Residents receive daily check-ins, intensive case management, and structured programming to build foundational life skills.

What's included

  • Daily case manager check-ins
  • On-site programming and life skills workshops
  • Structured daily schedule
  • 24/7 staff availability
  • Medication management support
Tier 2Transitional

Moderate-Support Housing

For men who have demonstrated stability and are ready for more independence. Case management continues on a regular schedule, with a focus on employment, financial literacy, and community integration.

What's included

  • Weekly case manager meetings
  • Employment and job readiness support
  • Financial literacy coaching
  • Community resource connections
  • Peer support programming
Tier 3Pre-independence

Low-Support Housing

The final step before independent living. Residents in Tier 3 are largely self-sufficient, with light-touch case management and a focus on securing permanent housing and long-term stability.

What's included

  • Bi-weekly case manager check-ins
  • Permanent housing search assistance
  • Benefits and entitlements navigation
  • Savings and budgeting support
  • Alumni network access

Supportive services

Supportive Services Across All Tiers

Regardless of housing tier, every resident has access to a core set of supportive services designed to address the whole person — not just their housing situation.

Case Management

Every resident is assigned a dedicated case manager who develops an individualized service plan, tracks progress, and connects residents to the resources they need.

Life Skills Training

Workshops and one-on-one coaching covering budgeting, cooking, communication, conflict resolution, and the practical skills needed for independent living.

Employment Support

Resume building, job search assistance, interview preparation, and connections to employer partners and workforce development programs.

Mental Health & Wellness

Referrals and coordination with licensed mental health providers, substance use counselors, and wellness programs in the community.

Benefits Navigation

Assistance applying for and maintaining public benefits including SNAP, Medicaid, SSI/SSDI, and other entitlements that support housing stability.

Community Integration

Connections to faith communities, volunteer opportunities, recreational programs, and peer support networks that build belonging and social capital.

Accountability

Structure That Supports Growth

Accountability is not a punishment — it is a framework for success. Our program sets clear expectations for residents at every tier, with regular check-ins, program agreements, and transparent processes for addressing challenges. We believe that structure, when paired with genuine support, creates the conditions for lasting change.

1

Clear Expectations

Every resident receives a written program agreement outlining expectations for conduct, participation, and progress milestones.

2

Regular Reviews

Formal progress reviews at 30, 60, and 90 days — and quarterly thereafter — ensure residents and staff are aligned on goals and next steps.

3

Transparent Processes

When challenges arise, we address them through a fair, documented process that prioritizes support and problem-solving over punitive responses.
